BOTH parents named on the minor's birth certificate.

ALL ADULTS will need:

* a valid U.S. passport
* OR a birth certificate and photo i.d. (social security cards, voters registration cards, photocopies of birth certificates, birth certificate cards or hospital certificates of birth are no longer accepted)

ALL MINORS will need:

* a valid U.S. passport
* OR a birth certificate and if the minor is 16 or older, a photo i.d. will be required (social security cards, voters registration cards, photocopies of birth certificates, birth certificate cards or hospital certificates of birth are no longer accepted)
* in addition, all minors will be required to have a notorized letter of permission signed by BOTH parents.
